This form will ask about your lifestyle habits (smoking, drinking alcohol, exercising, etc.), your family medical history, your personal health history, and more. It’s important to be as thorough as possible on this form—listing any present medications and health conditions—so that Dr. Moiceanu and our team can tailor your dental care to fit your unique health needs.
What Kind of Health Information Should I Share?
We believe that it’s better to have more information about you than we need, so we encourage you to tell us as much about your health as possible. Even the smallest detail can make a difference in the way we provide you with quality dental care, so, if it happened, we need to know about it.
Also, please don’t rely only on the registration form to tell us about your overall health. If you have a serious health condition—including heart problems, asthma, diabetes, if you’re pregnant, and more—we need to know about it when you take a seat in the dental chair. These conditions and others have a direct effect of the way Dr. Moiceanu and our team administers your dental care. Additionally, we need to know any medications that you’re currently taking to ensure that you will not have a reaction to any anesthesia or sedation that is used.
